Medical Records

Medical Records and/or letters from a provider are available upon request. When sending and requesting medical records it is required the patient or an authorized individual fills out a Release of Information authorizing us to send records. We require a minimum of 7-14 business day to prepare your records and/or write provider letters. Letter must be mutually agreed upon by the provider and patient before it is prepared and sent to the designated recipient.

There are administrative copying fees associated with the release of records for personal records. However, we do not place a charge for continuation of care. Please call our office at 509-420-5060 to talk to our medical records department for more information.
